Please download this document if have requested that your team sponsor plaques be individualized with each sponsor’s name. In order for us to ensure that every team plaque has the correct sponsor name on it, we need you to fill out these forms.

This page will include information for 5 teams – please print as many as necessary for your league. The fields “Team Name”, “Coach’s Name”, and “Uniform Color” are necessary for identification purposes, and the field “Sponsor Name (As you want it to appear on plaques)” is for the name of the sponsor exactly the way it will appear on the final plaques. Please make sure that this final line is spelled correctly and printed legibly.

Return all completed forms to the photographer on picture day, or simply drop them in the mail using the attached envelope.

If you would prefer to not have individualized plaques, but instead have each team plaque simply state “Thank You For Your Sponsorship”, simply call us at 800-852-0674 and let us know. If you decide to have generic plaques you do not need to fill out the attached pages.
